An at-risk resident is an individual whose physical, mental, or social condition increases their likelihood of experiencing injury, illness, neglect, or decline in health and well-being. This may result from factors such as advanced age, chronic medical conditions, cognitive impairment, limited mobility, social isolation, or a lack of support systems.
These residents often require closer monitoring, individualized care plans, and proactive interventions to maintain safety and quality of life.
Entering a resident as “at-risk” in the Frontline database will assist patrol officers in responding to calls for assistance. As a reminder: If you dial 9-1-1 due to an ongoing incident, it is important to advise the dispatcher that the individual is entered as an “at-risk” resident.

Additional Resource: You also can pick up a Vial of Life form (courtesy of the Bartlett Fire Protection District) at the Bartlett Police Department. The Vial of Life, also known as Vial of L.I.F.E. (Lifesaving Information for Emergencies), is a program that allows individuals to have their complete medical information readily available in their homes for emergency personnel to reference during an emergency. The program provides the patient's medical information when the patient is unable to speak or remember it.
